因为没有全文，并不能完全猜到意思。看上去好像是在为你们的设计项目做施工前的崭时停水申请。
"water distribution division" 这是每个地方都有的负责管理水的机构，有一点象中国的自来水公司。Provide water distribution at an economical price,hydrant maintenance,backflow/cross connection inspection,Install new meters,leak repairs,water main taps.
“field customer services”现场客户服务
“water division form”应该是这个机构的有关表格
请注意“the Water Distribution Division ”当每一个词前用大写时，通常指机构名称。
